Positive Notes is currently accepting ESA funds, DDD, private pay clients, and BCBS of Arizona.  
Private pay rates are $250.00 for an evaluation and $130.00 per hour for ongoing services.  Private pay allows Positive Notes to provide therapy that is specifically tailored to a child's needs without barriers and limits presented by insurance providers.  Positive Notes is considered out-of-network with most insurances but can provide you with a "superbill" which is an itemized receipt for out-of-network therapy.  Superbills can be submitted to insurance for reimbursement depending on your health insurance plan.  It is your responsibility to contact your insurance to verify if a superbill will be accepted prior to starting services.
